Request assistance here!

Fill out this confidential form and we will refer you to the best therapist for your needs in your area. The therapist will reach out to make you appointment, so please make sure your email and phone are correct. If you are applying for your children, we need a form for each child. WE CAN NOT ACCEPT FORMS FOR OTHER PEOPLE. They have to be ready to accept help and ask for it on their own.
